Immunization Records

At the time of enrollment, the parent/guardian must provide a record showing that the student has met the immunization (shot) requirements stated in Texas Government Code §38.001, proof that the student is not required to be immunized, or proof that the student is entitled to be admitted provisionally. Immunization records must have a physician's signature or stamp.

Note: All students attending 7th-12th grade must receive a meningococcal conjugate (MCV4) vaccine and one dose of the tetanus-diphtheria-acellular pertussis (Tdap) vaccine by state law.
