UIL Forms and Physicals
A new physical is required before participation each school year for all student athletes and cheerleaders in grades 7 through 12, as well as band and drill team members in grades 9 through 12. Please download and print the Physician's Statement Form to bring to your doctor's appointment.
All required UIL eligibility forms must be submitted each school year. You can access and submit these documents online through the UIL Student Forms Portal.
- Do not log in or create an account. Instead, select the option to Continue as a Guest.
- Begin with the first form, titled Splendora ISD Student Release, and complete all nine forms in order. Select Submit after completing each individual form. Once you submit the form, select the Next button to proceed.
- After completing all nine forms, you will see a completion message on your screen. You will also receive a separate email confirmation for each form you successfully submit. Keep these emails for your records to confirm completion.
Helpful Hints and Troubleshooting
Please enter your student's legal name exactly as it appears on their official school registration, including any hyphens. Do not use nicknames.
If your student's profile does not appear, try removing the leading zero from their student ID number. If they are a new student to the district, their profile may not be built yet. Please note that the athletic trainers must manually create profiles for new students.
Make sure to complete every field on all forms. Leaving blank spaces will prevent the forms from submitting. You do not need to print these documents. Your online completion and submission will be saved directly to our secure servers.
