Alternate Bus Stop Request Form

The Alternate Bus Stop Request Form is for requesting a permanent or long-term change to your child’s assigned bus stop address. Use it if your child needs to regularly get on or off the bus at a different stop (for example, a relative’s home or a daycare).

Important Information:

• This form is not for temporary or one-time changes.

• For short-term or daily changes, contact your child’s campus office to obtain a bus pass.

• All requests will be reviewed. Students can only be assigned to existing bus stops in accordance with the district’s walk-to-stop policy.

• New bus stops will not be created to accommodate alternate stop requests.
