Immunization Requirements for 7th Grade Students

All incoming 7th grade students are required by the State of Texas to have the following immunizations prior to entry:

1. 2 doses of varicella vaccine or a written statement from the parent or physician attesting to the child's positive history of varicella disease (chicken pox).
2. Meningococcal vaccine
3. Tdap booster vaccine if it has been 5 years since their last Tdap vaccine.

If you are not sure if your child has had the required immunizations, you may contact your child's campus or your physician.  Please forward documentation of immunizations to your child's campus upon receipt.
